CHRISTCHURCH SCHOOL SUMMER PROGRAMS Parent/Guardian Consent Form
(the "Camper")
This Parent/Guardian Consent Form addresses Camper’s participation in the 2025 Summer Programs at Church Schools in the Diocese of Virginia, Inc. d/b/a Christchurch School (School) (Summer Programs). The undersigned parents/legal guardians/other financially responsible parties (Parents), individually and collectively, represent that they have the authority to provide the consents below. Parents understand, acknowledge and consent to the following:
Summer Programs
The School provides opportunities for campers to engage in a wide range of programming while enrolled in the School’s Summer Program. Parents grant permission for Camper to participate in all aspects of the Summer Program and its activities (Activities). Parents understand that certain Activities may involve risk of injury or illness, and that Camper is participating in Activities knowing that the School cannot protect Camper from all associated risks, whether caused by Camper, others, or nature. Parents understand that Activities may involve travel and authorize Camper to be transported in a School-owned and operated vehicle, in a vehicle chartered by the School, in a vehicle owned or operated by a School employee or volunteer or on public transportation if any Activities are held off School property. Parents understand that Activities may be supervised by School employees, contractors, volunteers, vendors, or others. Parents understand that the School does not represent or act as an agent for, and cannot control the acts or omissions of, any volunteer, vendor, transportation carrier or other provider of food, goods or services involved in any Activities. Parents understand that the School is not responsible for matters that are beyond its reasonable control.
Medical Care
The School recognizes that Summer Programs campers sometimes require medical care while participating in Activities. This consent for medical care authorizes the School to obtain appropriate medical care on behalf of a camper who requires such medical attention. Parents authorize and appoint the School, through its designated agents and personnel, to provide and facilitate Camper’s medical care. Parents understand that this authorization includes medical care that is usual and customary for routine care, preventative care, athletic care, and emergency care, including care for injuries and illnesses and medication administration. Depending on the circumstances, the School may determine whether Camper is removed from Activities for a medical reason. Depending on the circumstances, the School may first try to contact Parents prior to facilitating medical care. In the event of a medical issue or emergency, including surgery requiring the use of an anesthetic, the School may act on behalf of Parents in granting permission for Camper’s medical care. In the absence of Parents from a medical facility offering medical care to Camper, Parents authorize the School to sign on behalf of Parents any authorization required by a medical provider or facility prior to or in the course of Camper’s care. Parents give the School, and its designated agents and personnel, permission to request, access, possess, and share medical documents and information regarding Camper, as well as to release medical information regarding Camper to any healthcare provider for medical reasons and to School employees and agents, as the School deems necessary. Parents will maintain United States-based medical insurance to cover medical costs, fees, and expenses associated with any medical care provided to Camper and will pay in full all costs, fees and expenses not covered by medical insurance relating to Camper. Parents authorize the School to communicate on behalf of Parents with healthcare providers and insurance companies regarding medical care for Camper, including associated medical costs, fees, and expenses.
Acceptance of Risk of Communicable Diseases
The School understands that Camper and Parents may bring personal belongings with them onto School property or to the School’s Summer Program Activities held on or away from School property. Such items may include personal electronic devices, sports equipment, jewelry, clothing and accessories, wallets, and other personal belongings that may or may not be of substantial value. Parents assume the responsibility for, and understand that the School is not responsible for, any personal belonging brought onto School property or to the School’s Summer Program and its Activities, whether held on or away from School property, by themselves or by Camper.
Use of Likeness
The School may record and use Camper’s name, likeness, image, voice and work in School-related communications and materials, including in any form or media.
Contact Information
The School may distribute Parents’ name and contact information to employees and parents of campers for School summer camp programming related use. Parents will not use or distribute the name or contact information of parents of campers enrolled in the School’s Summer Program for commercial or solicitation purposes.

(the "Camper")
This Enrollment Contract for the School’s 2025 Sports on the River summer camp programming (Contract) is entered into between Church Schools in the Diocese of Virginia, Inc. d/b/a Christchurch School (School) and the undersigned parents/legal guardians/other financially responsible parties, jointly and severally (Parents). In consideration of the terms below, the parties agree as follows:
Camper Reservation
The School provides opportunities for campers to engage in a wide range of programming while enrolled in the School’s Summer Program. Parents grant permission for Camper to participate in all aspects of the Summer Program and its activities (Activities). Parents understand that certain Activities may involve risk of injury or illness, and that Camper is participating in Activities knowing that the School cannot protect Camper from all associated risks, whether caused by Camper, others, or nature. Parents understand that Activities may involve travel and authorize Camper to be transported in a School-owned and operated vehicle, in a vehicle chartered by the School, in a vehicle owned or operated by a School employee or volunteer or on public transportation if any Activities are held off School property. Parents understand that Activities may be supervised by School employees, contractors, volunteers, vendors, or others. Parents understand that the School does not represent or act as an agent for, and cannot control the acts or omissions of, any volunteer, vendor, transportation carrier or other provider of food, goods or services involved in any Activities. Parents understand that the School is not responsible for matters that are beyond its reasonable control.
Payment
Fees and payment schedules for the Summer Program are identified on the Registration Form (Fees). Parents will pay Fees to the School in accordance with the payment schedule identified on the Registration Form.
School Authority
Parents and Camper are bound by all School rules, policies and procedures applicable to the Summer Program. The School may add to, modify, or remove its rules, policies, and procedures at any time. All decisions regarding Camper enrollment in the Program, conduct, financial account and the interpretation and enforcement of applicable School rules, policies and procedures are vested with the Head of School and/or the Head of School’s designated alternate, as well as with the Director of the School’s Summer Program and/or their designated alternate, at the Head of School’s absolute discretion. The School may discipline Camper if Camper’s conduct, or a Parent’s conduct, is deemed unsatisfactory by the School, up to and including separation of Camper from the Summer Program. Parents agree to pay for any damage to School property caused by Camper or Parents. Parents certify that they have provided the School all information about Camper relevant to Camper’s participation in the School’s Summer Program, such as information regarding mental, physical, emotional and health needs and challenges, as well as behavioral and disciplinary history. Parents agree to provide updates to relevant Camper information and to provide new information about Camper relevant to Camper’s enrollment in the School’s Summer Program as it materializes.
Force Majeure
The School will not be liable for any failure or delay in the performance of its duties or obligations under this Contract to the extent such failure or delay is caused by a force majeure or an event beyond the School’s reasonable control. Such events include but are not limited to fire, flood, earthquake, hurricane, tornado, other natural disaster, act of God, war, government action, act of terrorism, riot, epidemic, pandemic, viral outbreak, strike, labor dispute or other major upheaval which renders performance of this Contract impractical, illegal, impossible, or otherwise inadvisable. In such an event, the School’s duties and obligations under this Contract will be modified or suspended, at the School’s discretion, until such time as the School determines that it may safely and ably resume performance. The School will provide notification of such a suspension or modification by a reasonable time and method.
Cancellation
The School may cancel the Summer Program at its discretion for any reason, including for insufficient enrollment. In the event the School cancels the Summer Program, the School will notify Parents in advance and provide a refund of any fees paid toward Camper’s enrollment in the Summer Program.
